Claris Healthcare Remote Care Grant 2026

The Claris Remote Care Grant will award three Tennessee Rural Health Clinics $15,000 each to launch remote care programs.

The grant will cover the hardware, setup and implementation costs involved in setting up RPM and CCM programs designed to generate recurring revenues for the selected RHCs.

Rural Health Clinics Face Unprecedented Challenges

Rural Health Clinics are the backbone of primary care in many Tennessee communities, yet they face growing pressure to care for aging populations with increasingly complex health needs.

Persistent workforce shortages, rising operating costs, and barriers to accessing care make it difficult for clinics to deliver proactive care beyond the exam room. At the same time, Rural Health Clinics are expected to improve outcomes, enhance patient engagement, and remain financially sustainable.

Financial
Pressures

Rural Health Clinics operate on tight margins while caring for aging populations with increasingly complex health needs.

Rising labor costs, workforce shortages, and growing demand for chronic disease management continue to strain clinic resources.

Chronic Disease
Management Gaps

Rural communities face higher rates of chronic conditions like hypertension, diabetes, heart failure, and COPD.

Traditional care models often lack the necessary support between visits, making it difficult to prevent avoidable utilization.

Staffing and
Workforce Challenges

RHCs continue to face persistent workforce shortages, making it difficult to recruit and retain physicians, nurse practitioners and care coordinators.

Smaller teams must care for growing patient populations while managing an expanding range of clinical responsibilities.

 Access Barriers for
Rural Patients

The vast geography of rural Texas forces patients to travel extensive distances for both routine and specialized care.

These long commute times create significant access barriers, often delaying critical interventions and discouraging necessary follow-ups.

Program Goals

Through this grant, we aim to help Rural Health Clinics solve these problems by deploying remote care programs that can deliver better care for their patients while generating new revenue streams

Benefits for Rural Health Clinics

$15,000 Grant Funding

Offset the startup costs often associated with launching a remote care program.

No Limit on Number of Patients Served

The initial goal will be to enroll a cohort of 100 patients. The long terms goal is to help RHCs enroll as many chronic care patients as possible.

Generate Recurring Revenue (and Profitability)

RPM and CCM services generate monthly Medicare reimbursement opportunities for RHCs.

Strengthen Rural Care Delivery

Extend care beyond RHC walls while improving patient engagement, especially in areas with great distances between patients and RHCs.

Align with Future Rural Health Funding Initiatives

CMS’s Rural Health Transformation Program specifically identifies remote monitoring and technology-enabled care as priority investments for rural healthcare organizations. This pilot provides RHCs with an opportunity to demonstrate experience in these programs.

Eligibility

To be eligible for this grant, your RHC must:

Serve at least 100 insured patients (Medicare or Medicaid)

Beyond The Grant (Sustainability)

The Claris Remote Care Grant is designed to help hospitals overcome the biggest barrier to adoption: startup costs. Once the pilot is established, hospitals can continue growing and sustaining their programs through Medicare reimbursement for eligible RPM and CCM services.

Rather than creating a temporary project, the goal is to establish a permanent remote care service line that:

  • Generates recurring reimbursement revenue
  • Supports chronic disease management
  • Improves patient outcomes
  • Expands rural healthcare access
  • Strengthens long-term hospital sustainability

The grant provides the launchpad. CMS reimbursement supports the future. Medicare continues to reimburse RPM and CCM services, and recent policy updates have expanded opportunities for rural providers to participate in remote care management programs.
