Quantify Specialty Care Expands Remote Care Access for High-Risk and Underserved Populations

Bringing care into the homes of patients who need it the most

For many patients living with chronic and complex health conditions – particularly those in rural or underserved communities, managing chronic health conditions can be challenging. Geographic isolation, limited transportation, and low digital literacy often prevent patients from consistently engaging with healthcare programs.

Quantify Specialty Care (QSC), a provider of specialty infusion therapies and remote monitoring services for patients with complex chronic conditions, partnered with Claris Healthcare to extend care directly into patients’ homes.Beyond remote monitoring and engagement, QSC supports a broad range of specialty infusion and injection therapies across multiple complex disease states. Internal operational data reflects experience with neurology, immunology, rheumatology, gastroenterology, oncology, and supportive care therapies, including Ocrevus, Entyvio, infliximab products, IVIG therapies, Keytruda, Tysabri, Rituxan, Benlysta, iron therapies, and others. This breadth reinforces that the QSC model is not limited to a single age group or condition category, but is built to support patients with complex clinical needs across diverse populations.

Through the program, patients received a complete remote care toolkit that included vital sign monitoring devices such as blood pressure cuffs, weight scales, blood glucose monitors, and pulse oximeters. Patients without smartphones received easy-to-use tablets with built-in connectivity, while others used SMS text messaging or the Claris Companion smartphone app to access care programs, messaging, and educational content.

Remote care staff supported patients throughout the program, helping them collect vital readings, stay on track with care plans, and remain connected to their care teams.

The result was a highly engaged remote care population managing their chronic conditions from the safety and comfort of home.

Patient engagement through QSC and Claris Healthcare - Remote Care

Serving Patients Across Rural and Underserved Communities

The patient population served by QSC represented a geographically diverse group of patients living in rural areas, many of whom face barriers such as limited transportation, mobility challenges, or low digital literacy.

Most patients were located across the southern United States, with Tennessee representing 38 percent of participants and Mississippi 23 percent. Smaller patient groups were also located in several other states across the country. These regions often face challenges such as long travel distances to healthcare facilities, shortages of healthcare providers, and inconsistent internet connectivity.

Women represented the majority of the cohort, accounting for 64 percent of participants, which aligns with common demographic patterns seen in chronic disease populations.

Many participants were living alone with limited mobility, or had limited experience using digital tools. For these patients, remote care programs must do more than provide technology – they must simplify healthcare engagement and make participation accessible.

Managing Complex Chronic Conditions at Home

Patients enrolled in the program represented a high-acuity population managing multiple chronic health conditions.

Some of the most common diagnoses included Hypertension, Type 2 Diabetes, Anxiety disorders. Other prevalent conditions included hyperlipidemia, depression, COPD, obesity, chronic pain, hypothyroidism, and sleep apnea.

Many patients managed several conditions simultaneously, highlighting the importance of consistent monitoring and ongoing communication between patients and care teams.

Health Management Through Patient Engagement

Patient engagement was one of the strongest indicators of the program’s success. Beyond clinical monitoring, patients accessed a range of educational and wellness content through the platform’s digital content library. Members engaged with articles, health resources and videos designed to help them better understand and manage their conditions.

Across the reporting period (2025), the platform logged more than 180,000 total activities, with 98 percent of enrolled members demonstrating calendar activity during the year, indicating exceptionally high platform engagement.

Vital sign monitoring represented the largest portion of platform activity, with over 96,000 vital readings recorded across 94 percent of members. Medication tracking and reminders also supported daily care routines, helping patients remain consistent with their treatment plans.

Medication adherence remained strong throughout the program with 86 percent of medication reminders that were completed.

Care Delivery Supported by Care Team Members

While the Claris Companion platform enabled remote monitoring and communication, the program’s success was equally driven by the care staff supporting patients throughout their care journey.

Care staff assisted patients with monitoring devices, helped troubleshoot technical issues, and maintained regular communication to ensure patients remained engaged with their care plans.

Messaging played a key role in maintaining communication between patients and care teams with over 100,000 messages exchanged between the patients and their care staff over the course of a year

Care team members maintain comprehensive clinical notes on QRC members. In total, 161 member notes were created (approximately 1 note per member on average), with extensive use of structured tagging for clinical governance.

Our care staff demonstrated strong operational efficiency, successfully reviewing and closing 100% of alerts generated from patient monitoring events, ensuring timely follow-up on every potential health risk.

Integrated AI documentation tools enhanced the accuracy and consistency of clinical notes by guiding care staff with structured inputs and real-time validation. These tools also generated intelligent suggestions for patient follow-up and next steps, enabling more informed, timely interventions and improving overall care quality.

This ongoing support helped integrate healthcare activities into patients’ daily routines while reinforcing adherence to monitoring programs and treatment plans.

For many seniors living alone or with limited mobility, this consistent connection to a care team created a sense of reassurance that their health was being actively monitored between clinical visits.

Capturing the Full Patient Story Through Surveys

Considering the rural and vulnerable nature of these patients, the program incorporated surveys designed to capture insights beyond clinical readings.

Throughout the program:

  • 5,566 survey forms were delivered
  • 100 percent of members were surveyed
  • 44 percent of surveys received responses

The surveys tracked symptoms such as pain levels, cough, fever, fatigue, and shortness of breath, as well as lifestyle factors including exercise participation.

The program also included Social Determinants of Health (SDOH) assessments exploring housing stability and food security. Among members who responded to these questions, nearly all reported stable housing and food access. Some examples of these questions include:-

These surveys allowed caregivers to gain deeper insights into patient wellbeing, enabling earlier intervention when symptoms or risks appeared.

Conclusion

The Quantify Specialty Care program demonstrates how remote care programs can successfully support patients living with complex chronic conditions across diverse populations.

quote from QSC

Patients who may have otherwise struggled to remain connected to healthcare providers were able to participate in regular monitoring, receive guidance from care teams, and access educational resources that supported long-term health management.

By creating a complete remote care system that is designed for some of the most challenging patients in the country, Claris Healthcare has created a care model that enables patients who may have otherwise struggled to remain connected to healthcare outside the clinic.
